How to Use the HEA Courses
To
optimize your investment in this continuing education experience, we
suggest that you take advantage of all materials included. We recommend
that you read the printed text first and then review the videotape/DVD,
using both media will maximize your retention of the information
presented. After you have completed the program of study,
complete the exercises at the end of each lesson along with the case
studies. Then turn to the section titled ‘Critique’ to evaluate
your performance.

You will find a
‘Course Examination’ test with an answer sheet. Please circle the
correct answer, only on the answer sheet. This is a separate test
that must be returned to us.
To apply for
your CEU certificate, fill out the double-sided page titled ‘Affidavit
of Completion and Course Critique’. You cannot be awarded your
CEU’s unless HEA receives the ‘Affidavit of Completion’ and the
one-page answer sheet. The answers are graded on a pass/fail basis. In
the event that you do not pass the test, we will notify you by phone
and mail. If you retake the course, there will be an additional
charge of $25.00. Please fill out all information on your
Affidavit, including the date of completion (e.g., June 21, 2007), your
name, address, license number and state where the license is
held. Include your daytime phone number, and if you want your CEU
certificate faxed to you, be sure to include your fax number. We
make every effort to fax your certificate within twenty-four hours.
All courses are registered in the name of the purchaser and may be used only by that person unless additional Affidavits were purchased. You will need to include the purchaser's customer number (found on their invoice) on your Affidavit of Completion form, or your certificate will not be issued. If the course was purchased by your facility then you must also include a customer number, (found on the invoice) or your certificate will not be issued. Please check with your facility if additional Affidavits were purchased along with the course, or a fee will be due upon completion.
